Re:  Modified Form 180 Records Request

 

Dear Web Site User:

 

We have made available our modified From 180 for you to use in obtaining your records.  This form will get you both service and medical records and a complete review and re-issuance of all personal and unit awards and decorations.

 

If printed on a color printer, our copy has yellow highlight.  The yellow highlighted areas require you to fill in your personal information.  Please use our on-line form to help you complete your form 180 request.

 

The procedure that we suggest using is as follows:

 

1.       Fill out the worksheet as indicated in our example.

2.      The finished and final form should be typed and sent to the National Personel Records Center in St. Louis, MO.  Their address is as follows:

When you begin to receive the records, please keep each set of requested records in a separate folder.  For example, keep your Military Service Record in one folder, your Military Medical Records in another folder, and so on. 

 

It takes some time for these records to be pulled, copied, and forwarded to you.  In addition, copies of this Form 180 request will be sent to various government offices to obtain the information, which you are requesting.  You may receive requests for additional information, or the records themselves, from various offices, including Headquarter Marine Corps.
